PRIVACY COLLECTION STATEMENT

The program is administered by AstraZeneca Limited (company number 14737) ("AstraZeneca", "we", "us" or "our"). AstraZeneca complies with New Zealand's Privacy Act 2020 (“Privacy Act”) in how we collect personal information about you, to ensure that your personal information is protected. In this statement, all references to personal information include sensitive information (such as information about your health).
 

During the program, AstraZeneca may collect the following personal information from you:

  • your phone number;
  • information about symptoms you experience and your risk factors for certain diseases; and
  • any other information that you choose to provide us with.


AstraZeneca will collect and use your personal information in order to enrol you in, administer and perform services relevant to the program (including providing you with materials, services and information you request and contacting you for follow-up purposes related to the program, such as to prompt you to visit your health care provider, to generate aggregated and anonymised statistics and insights about the symptoms and risk factors experienced by the participants in the program, and for purposes otherwise set out in AstraZeneca's Privacy Policy, available at this link.


The information you provide will be collected by or on behalf of us and may be shared with  third party service providers who assist us to provide the program, including Adobe Campaign (which stores data in Dublin, Ireland). AstraZeneca requires these persons or organisations to maintain the confidentiality of your personal information and to only use that information for the services they perform. We may also disclose your personal information including information you provide us about symptoms you experience and your risk factors for certain diseases to our related entities, in order to carry out the purposes described above, and (if applicable) to report adverse events as further described below. AstraZeneca continually monitors the safety profile of their products. To support this ongoing process, and to adhere with local and global regulatory obligations we must collect all reports of potential Adverse Events (AE), special situations, and product quality complaints, in relation to a patient potentially administered an AstraZeneca product. If we identify a potential report during our communications with you, this will be shared with AstraZeneca’s medical (pharmacovigilance) team.  The information may be disclosed to local and overseas regulatory authorities or other third parties (such as those providing AE case processing activities, or license partners), for the purposes of meeting pharmacovigilance requirements. If you provide your contact details to us, you consent to us using those details to contact you for further information on the AE reports. In the event that you report an adverse event to one of our products, AstraZeneca will seek your consent to disclose any information you provide at the time of reporting, including any sensitive information, to each other and to your healthcare professional in order to notify and follow up with your healthcare professional.  AstraZeneca will also seek your consent to obtain further information about you from your healthcare professional and from yourself including, information about your health condition, the symptoms you experienced, other relevant information about your medical history and the medications you take. AstraZeneca will use any additional information collected about you from your healthcare professional only for the purposes of recording, analysing and monitoring adverse events to our products and improving patient safety.  Your consent is completely voluntary and will not affect your participation in the program in any way.


AstraZeneca may transfer your personal information overseas, as explained above. In the event that AstraZeneca discloses your personal information outside New Zealand, AstraZeneca will comply with the requirements of the Privacy Act that relate to transborder data flows.


The Privacy Policy of AstraZeneca (available at this link) contains further information about how we store and use and how you may exercise your rights to access and/or correct the personal information held about you as required by law, as well as information about making a complaint about a breach of the Privacy Act and how AstraZeneca will deal with such a complaint. Unless you inform us otherwise, by submitting this form, you consent to AstraZeneca collecting, using and disclosing your personal information in this way. If you do not provide the information to us, you may be unable to participate in the program.
